Position Summary


The Speech-Language Pathologist is responsible for evaluating, developing, and implementing individualized treatment plans for patients receiving home health services. The SLP provides skilled therapy services in accordance with physician orders, the patient’s Plan of Care, and applicable federal, state, and agency regulations.


Essential Job Responsibilities


  •  Conduct comprehensive speech-language, cognitive, and swallowing evaluations 
  •  Assess patient functional status and home environment as appropriate 
  •  Develop individualized treatment plans based on physician orders 
  •  Provide skilled speech therapy interventions to improve communication and swallowing function 
  •  Recommend communication devices and adaptive equipment as needed 
  •  Educate patients, families, and caregivers regarding treatment plans and home programs 
  •  Collaborate with physicians and interdisciplinary team members to coordinate care 
  •  Participate in case conferences and care planning meetings 
  •  Complete timely and accurate clinical documentation per agency policy 
  •  Monitor patient progress and update treatment plans as needed 
  •  Identify additional patient needs and coordinate appropriate referrals 
  •  Supervise Home Health Aides when services relate to the therapy plan of care 
  •  Maintain clinical competency through ongoing education and training 
  •  Perform additional duties as assigned to support patient care delivery 
Qualifications


Required


  •  Master’s Degree in Speech-Language Pathology 
  •  Current California Speech-Language Pathologist License 
  •  Current CPR/BLS Certification 
  •  Valid California Driver’s License 
  •  Reliable transportation and current auto insurance 
  •  Strong communication and documentation skills 
  •  Ability to work independently and within an interdisciplinary team 
Preferred


  •  Home Health experience 
  •  Experience with Medicare documentation requirements 
  •  Experience using Electronic Medical Records (EMR) systems
