Role Summary

Premier Therapy is hiring a PT Physical Therapist to provide part-time rehab services in a partner skilled nursing facility in Wauseon, OH—supporting residents as they regain mobility, strength, and day-to-day confidence. You’ll complete evaluations, deliver individualized treatment aligned to the plan of care, track outcomes with timely documentation, and collaborate with the interdisciplinary team to promote safe, functional progress.

Qualifications:
- Current Ohio Physical Therapist license and required credentials
- Skilled nursing/geriatric rehab experience preferred
- Solid clinical judgment, clear communication, and strong documentation habits
