Role Overview We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador.
Post-discharge follow-ups, med reconciliation, labs/diagnostics
Coordinate with facility staff, specialists, families/POAs
Same-day documentation; CMS/regulatory compliance
Schedule & SettingPart-time position with a structured weekday schedule. You'll spend 3 days per week delivering care, with some days on-site across Stockton, Modesto, and Amador (strategically clustered routes designed to minimize travel and maximize time with patients) and other days via telehealth.
Requirements
Qualifications
Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
Geriatrics or facility-based experience preferred; EMR proficiency
Benefits
Compensation
Annual compensation range: $115,000–$130,000 (based on experience, schedule, and patient volume)
Additional earning opportunities available based on coverage and panel needs
W-2 with mileage reimbursed per policy
Final terms outlined in the employment agreement
