Pathways to Community is a collaborative partnership between the Coconino County Sheriff's Office and Coconino County Health and Human Services (CCHHS) that provides support for individuals entering and leaving the Coconino County Jail. Individuals being released from custody receive immediate connection to social, behavioral, housing, transportation, and legal services.
The Pathways to Community - Community Health Worker will be housed at the Pathways to Community reentry site located adjacent to the Coconino County Sheriff's Office Complex, with limited access to the detention facility. This position will be responsible for conducting screenings with clients to identify needs, assisting with reentry resource navigation, making appropriate referrals, and attending court hearings. The Community Health Worker will participate as a member of the on-site Pathways team and play an essential role in supporting individuals throughout the reentry process.
COMMUNITY HEALTH WORKER I
Under direct supervision, performs work of moderate difficulty in community re-entry and community health work for the Pathways to Community Program. Conducts all client screenings for Pathways, performs resource navigation work, outreach to promote, maintain, and improve individual and community re-entry success; performs related work as assigned.
COMMUNITY HEALTH WORKER II
Under general direction, is responsible for work of moderate difficulty in performing professional community re-entry and community health work for the Pathways to Community Program; performing related work as assigned.
Please Note: This is a Limited Appointment position funded by the Pathways to Community Legislative Fund and is set to end June 30, 2027.

Essential functions of this position include but are not limited to: driving; working with and around others, co-workers, clients, agencies, etc.; working alone; dealing with interruptions; repetitive motion (hand/wrist) for computer input and driving; vision acuity (near/far); hearing and speech for ordinary conversation; touch/finger dexterity for computer work.
Minimum Qualifications
COMMUNITY HEALTH WORKER I
Two years of experience in a directly related role serving a community-based or vulnerable populations; OR any equivalent combination of education, training and experience which demonstrates the ability to perform the duties of the position.
ADDITIONAL REQUIREMENTS:
Must obtain an Arizona drivers license by date of hire and maintain it throughout employment. Final job offer will be contingent upon a satisfactory 5-year Arizona Motor Vehicle Report. Fingerprint clearance may be required.
COMMUNITY HEALTH WORKER II
Three years' experience in a directly related role serving a community-based or vulnerable populations; OR any equivalent combination of education, training and experience which demonstrates the ability to perform the duties of the position.

Must obtain an Arizona drivers license by date of hire and maintain it throughout employment. Final job offer will be contingent upon a satisfactory 5-year Arizona Motor Vehicle Report. Fingerprint clearance may be required.
This position is safety sensitive.
This position is non-exempt from overtime.
PREFERRED QUALIFICATIONS:
Experience, including professional OR personal lived experience navigating behavioral health/substance use/criminal justice systems. A combination of professional and lived experience, an awareness of resources available to justice involved individuals, and a passion for serving this community.
The ideal candidate will have experience working respectfully with individuals facing significant challenges, including people experiencing homelessness, behavioral health needs, and those involved with the criminal justice system. They should be well-connected to the community and have a genuine passion for supporting justice-involved individuals. This candidate will bring meaningful lived experience that relates to the role and be willing to share that experience appropriately with clients, helping them build strong, trusting connections with the people Pathways serves. They should be compassionate, conscientious, detail-oriented, and reliable. The ideal individual will take initiative, demonstrate a strong work ethic, be accountable, and collaborate effectively with others, including County departments and community partners.
