Nature of Work

Pathways to Community is a collaborative partnership between the Coconino County Sheriff's Office and Coconino County Health and Human Services (CCHHS) that provides reentry support to individuals transitioning from the Coconino County Detention Facility back into the community. The Pathways to Community Front Desk Resource Specialist position will serve as the primary point of contact for clients at the front desk at the Pathways to Community program. This position will engage with clients as they are released, provide basic resource navigation that can be addressed at the front desk, and triage individuals to the appropriate staff or community resource based on their identified needs. The position will conduct client screenings as needed and serve as a backup to the Community Health Worker team to support client engagement, resource navigation, and other CHW-related duties. The position will also provide administrative support to the Pathways team and will operate on site with no remote work opportunities.

FRONT DESK RESOURCE NAVIGATOR I

Under direct supervision, performs work of moderate difficulty in community re-entry and community health work for the Pathways to Community Program. Conducts all client screenings for Pathways, performs resource navigation work, outreach to promote, maintain, and improve individual and community re-entry success; performs related work as assigned.

FRONT DESK RESOURCE NAVIGATOR II

Under general direction, is responsible for work of moderate difficulty in performing professional community re-entry and community health work for the Pathways to Community Program; performing related work as assigned.

Typical Duties

FRONT DESK RESOURCE NAVIGATOR I

  • Serves as a primary point of contact with Pathways to Community clients as they are booked into the Coconino County Detention Facility and engages clients as they are released and receiving services at Pathways
  • Conducts screenings on individuals booked into the Coconino County Detention Center to determine their needs and inform the development of individualized reentry plans
  • Monitors and tracks progress of individuals in the program and adjust services as needed to ensure success
  • Documents all client interactions and service delivery progress in accordance with program policy and protocol
  • Works to improve health equity and resource availability for clients by participating on Pathways service team, attending case staffing's and collaborates with other Pathways team members.
  • Conducts outreach to clients for post-release follow-up
  • Serves as a liaison between Pathways to Community, Pathways partners, clients, and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery.
  • Works closely with community partners to provide a seamless network of support services to individuals
  • Maintains accurate and complete records of all program activities
  • Remains informed on best practices and emerging trends in reentry programs and services
  • Works closely with teams of professionals to assess the needs of each individual and provide a range of support services to help them successfully reintegrate into their communities.
  • Identifies or contacts members of vulnerable populations
  • Interprets, translates, or provides cultural mediation related to health services or information for community members
  • Provides feedback to Health and Human Services programs regarding improving service accessibility or acceptability
  • Refers community members to needed health and social services or other identified needs; assists community members in navigating systems of care
  • Provides health education or otherwise disseminates medical, behavioral, or dental health information to school groups, community groups, or targeted (identified/prioritized) families or individuals, in a manner consistent with cultural norms
  • Maintains updated client records with plans, notes, appropriate forms, or related information
  • Advocates for individuals or community health needs with government agencies or health services providers
  • Attends community meetings or health fairs to understand community issues, build relationships with community members, and provides resources to care and services
  • Contacts clients in person, by phone, or in writing to ensure they have completed required or recommended actions
  • Distributes flyers, brochures, or other informational or educational documents to inform members of a vulnerable community
  • Performs related duties

FRONT DESK RESOURCE NAVIGATOR II

  • Performs the duties of a Community Health Worker I
  • Provides administrative support to Pathways Manager as assigned, including supporting case staffing management and chart review as assigned
  • Remains informed on best practices and emerging trends in reentry programs and services
  • Conducts outreach to community resources to partner in proving trainings and support to clients
  • Provides resource navigation for self-directed client re-entry, or otherwise disseminate medical, behavioral, or dental health information to school groups, community groups, or targeted (identified/prioritized families) or individuals in, in a manner consistent with cultural norms.
  • May be responsible for the development of additional trainings are required for the population(s) served
  • Complete quarterly file reviews and post-screening report audits

Essential functions of this position include but are not limited to: driving; working with and around others, co-workers, clients, agencies, etc.; working alone; dealing with interruptions; repetitive motion (hand/wrist) for computer input and driving; vision acuity (near/far); hearing and speech for ordinary conversation; touch/finger dexterity for computer work.

Minimum Qualifications

FRONT DESK RESOURCE NAVIGATOR I

Two years of experience in a directly related role serving a community-based or vulnerable populations; OR any equivalent combination of education, training and experience which demonstrates the ability to perform the duties of the position.

ADDITIONAL REQUIREMENTS:

Must obtain an Arizona drivers license by date of hire and maintain it throughout employment. Final job offer will be contingent upon a satisfactory 5-year Arizona Motor Vehicle Report. Fingerprint clearance may be required.

FRONT DESK RESOURCE NAVIGATOR II

Three years' experience in a directly related role serving a community-based or vulnerable populations; OR any equivalent combination of education, training and experience which demonstrates the ability to perform the duties of the position.

ADDITIONAL REQUIREMENTS:

Must obtain an Arizona drivers license by date of hire and maintain it throughout employment. Final job offer will be contingent upon a satisfactory 5-year Arizona Motor Vehicle Report. Fingerprint clearance may be required.

The ideal candidate will have a deep connection to the justice involved clients that Pathways serves. This candidate will have some form of lived experience that allows them to connect with clients, and will be committed to engaging clients who are leaving incarceration to help support their successful re-integration into community. The ideal candidate will be familiar with the challenges that individuals leaving incarceration face, and will be enthusiastic about maintaining a positive, welcoming environment at the Pathways site, helping clients navigate the services available, assisting with basic need connection and supporting the Pathways team through front desk administrative support.

This position is safety sensitive.

This position is non-exempt from overtime.
