Starting Pay Rate Range: $16.42 - $19.74, commensurate with experience

Summary

Responsible for ensuring that patients receive high-quality, personalized care while navigating the healthcare system. This role focuses on enhancing the patient experience by addressing concerns, facilitating communication between patients and healthcare providers, and ensuring access to essential medical services.

Principal Functional Responsibilities

Patient Support and Records: Assist patients with scheduling appointments, referrals, and follow-ups. Serve as the primary point of contact for patients, ensuring they understand their care plans, treatment options, and available resources. Advocate for patients by addressing concerns, resolving issues, and providing education on healthcare rights and benefits. Maintain accurate records of patient interactions and track progress in care coordination.

Patient Education: Ensure patients have access to necessary medications, treatments, and support services. Educate patients on preventive care, wellness programs, and chronic disease management.

Collaboration: Collaborate with all healthcare providers, consultants, outside medical offices, hospitals, insurance companies, and social service agencies to coordinate care.

Quality Assurance: Support initiatives aimed at improving patient satisfaction, engagement, and health outcomes.

Other Duties: Perform other duties as assigned.

Qualifications

  • High school diploma or equivalent
  • Minimum one year of experience in a healthcare, social work, or customer service role
  • Knowledge of healthcare systems, insurance processes, and patient rights

Preferred Qualifications

  • Associate's or Bachelor's degree in healthcare, social work, or a related field
  • Prior experience in patient advocacy or care coordination
  • Proficiency in medical terminology and electronic health records (EHR)

Physical Requirements

Must be able to utilize a phone, computer and other office equipment. Must be able to occasionally lift to 25 pounds.

May be required to assist with clinical operations at other clinical sites when necessary.
