Patient Care Advocate

Pay Range: $27/hr - $30/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Conduct high-volume outbound calls to Medicaid members.
• Educate members on the importance of preventive care, screenings, and routine healthcare.
• Identify and help close member care gaps.
• Schedule and confirm approximately 14–16 appointments per day.
• Address member questions and concerns, coordinating with other departments as needed.
• Maintain accurate documentation of member interactions and follow-up activities.
• Consistently meet or exceed productivity and outreach goals, including 95 outbound calls per day.
• Deliver a positive and professional member experience during every interaction.
• Adapt quickly to new systems, processes, and company changes.
• Work independently while effectively managing time and daily priorities in a remote environment.

Key Requirements and Technology Experience: 

• 4+ years of total work experience required.
• 2+ years of experience in direct patient care, social work, quality improvement, health coaching, or a related healthcare environment.
• Previous experience handling a high volume of outbound calls.
• Experience with patient/member outreach or engagement.
• Strong communication and customer service skills.
• Ability to work independently and consistently meet measurable productivity goals.
• Previous remote work experience is required.
• Strong computer skills and the ability to quickly learn new systems and processes.
• Experience working in a managed care or health plan environment.
• Knowledge of HEDIS, care gaps, preventive care, or population health.
• Experience working with Medicaid members.
