Patient Care Advocate

Summary:

Work Mode: 100% REMOTE - Nationally sourced but must work PST hours

Duration: 2 months, possible extension

SHIFT: 8 hours per day M-F 8-5 PST; No overtime

Responsibilities:

• Make outreach calls to members, educating them on the importance of preventative care and screenings.
• Address all member needs and create positive experiences.
• Complete member outreach calls to close care gaps for Medicaid.
• Work with other departments to resolve member concerns.
• Achieve outreach call goals of above 95 calls per day and 14-16 appointments made/confirmed per day.

Requirements:

• Bachelor’s degree in healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related health field or equivalent work experience.
• Minimum 4 years of experience in direct patient care, social work, quality improvement, or health coaching, preferably in a managed care environment.
• Basic computer skills test - intermediate level.
• Previous experience in a role handling a high volume of outbound calls.
• Ability to work effectively in a remote environment while remaining self-motivated and driven to meet or exceed established goals.
• Ability to learn and adapt to new systems and company changes.

Preferred Skills:

• Member outreach experience.
• HEDIS knowledge.
• High volume outbound call experience.
