Patient care assistant – FULL-TIME POSITION OPEN!

OverviewWe are seeking a dedicated Patient Assistant Night Shift to join our healthcare team.

Starting pay Rate – $18 -$ 20 per hour

The ideal candidate will have experience in a nursing home, community care, or a teaching hospital setting. This position offers an opportunity to work in the patient’s home environment.

Responsibilities- Assist patients with activities of daily living, personal care- Monitor and record vital signs- Provide support in in-home and with errands, appointments and activities outside of home, such as shopping and light housekeeping - Assist in medical documentation and record-keeping- Collaborate with the patient, family, and members of our home health agency to ensure patient well-being

Skills- Experience in nursing home or home community care preferred- Familiarity with basic medical documentation- willing to receive on the job training if no history in-home care or clinical experience- Knowledge of dementia care practices- Strong communication and interpersonal skills, basic phone and computer skills
