Patient Care Coordinator - Adult Therapy

Every phone call you answer, appointment you schedule, and patient you assist helps make a meaningful difference in someone's rehabilitation journey.

What You'll Do

As the hub of our Adult Therapy department, you'll help ensure patients receive outstanding care by:

  • Greeting patients and creating a welcoming, positive experience from the moment they arrive.
  • Scheduling evaluations, treatments, and follow-up appointments while keeping therapist schedules organized and efficient.
  • Answering incoming calls and assisting patients, families, physicians, and referral sources with professionalism and compassion.
  • Verifying referrals, obtaining authorizations, and ensuring patient information is complete and accurate.
  • Managing appointment reminders, follow-up communications, and rescheduling missed visits.
  • Maintaining accurate patient records, attendance logs, and departmental documentation.
  • Supporting therapists with clerical and administrative tasks that help them focus on patient care.
  • Collaborating with team members to solve scheduling challenges and keep daily operations running smoothly.

We're looking for someone who is:

  • Friendly, compassionate, and committed to delivering outstanding customer service.
  • Highly organized with exceptional attention to detail.
  • Able to manage multiple priorities in a busy clinical environment.
  • An excellent communicator with patients, coworkers, physicians, and referral sources.
  • Comfortable using computers and learning new software systems.
  • Experienced with general office procedures (minimum of two years of clerical experience required).
  • Proficient with Microsoft Windows and Word (preferred).
  • Comfortable working with individuals who may be ill, disabled, or emotionally distressed.
  • Able to lift up to 25 pounds and perform the physical requirements of the position.
