Mindify Wellness and Care is seeking an outgoing, compassionate, and relationship-focused Patient Care Coordinator to serve as a trusted community representative within our adult day care center partnerships throughout New Jersey.

In this role, you will help bridge the gap between individuals in need of behavioral health services and the care they deserve. As a vital member of the Mindify team, you will educate participants about our mental health services, foster meaningful relationships with patients and caregivers, support continuity of care, assist with interpretation during therapy and psychiatric appointments when needed, and help connect individuals to appropriate behavioral health resources.

Work Location & Schedule

This is a part-time position based within our partner adult day care centers throughout New Jersey.

• Travel to assigned adult day care centers throughout New Jersey.
• Typical schedule is Monday through Friday, generally between 8:30 AM and 2:30 PM.
• Hours are typically up to 30 hours per week.
• Daily schedules may vary based on center needs, outreach activities, patient engagement opportunities, and travel requirements.
• Some days may be shorter than six hours.
• Flexibility and adaptability are important in this community-based role.
• Mileage reimbursement is available for travel between centers when visiting multiple locations on the same day.

Key Responsibilities

Patient Engagement

• Build strong, trusting relationships with adult day care participants, caregivers, and center staff.
• Educate participants about Mindify's psychiatric, therapy, and behavioral health services.
• Serve as a welcoming and accessible resource for individuals seeking information about mental health care.
• Encourage participation in behavioral health services and promote overall wellness.

Education

• Educate participants, families, and center staff about Mindify's psychiatric, therapy, and behavioral health services.
• Serve as a knowledgeable resource regarding available treatment options and support services.
• Answer questions and help individuals understand how to access behavioral health care.
• Participate in community outreach and educational activities as needed.
• Deliver informational presentations or wellness discussions on an occasional basis to a group of 60+ when requested by your supervisor
• Help increase awareness of behavioral health resources and reduce barriers to care within the communities we serve.

Care Coordination & Patient Advocacy

• Identify individuals who may benefit from behavioral health services and facilitate referrals.
• Help patients navigate available services and resources.
• Support continuity of care by assisting patients with appointment coordination and ongoing engagement in treatment.
• Advocate for patients by helping them understand available treatment options and access appropriate support.
• Serve as a liaison between patients, families, providers, adult day care center staff, and Mindify clinical teams.
• Ad hoc assignments as assigned

Relationship Management

• Maintain strong working relationships with adult day care center administrators, activity staff, nursing staff, and other key stakeholders.
• Foster collaborative partnerships that support patient care and service growth.
• Represent Mindify professionally within the centers.

Translation & Interpretation Support

• Provide Spanish-English interpretation and translation services for patients, families, and providers when needed.
• Assist with communication during therapy and psychiatric appointments to help ensure patients receive culturally responsive and accessible care.

Documentation & Compliance

• Maintain accurate records of patient interactions, referrals, presentations, and outreach activities.
• Complete required documentation in a timely and professional manner.
• Maintain HIPAA compliance and patient confidentiality at all times.
• Perform additional patient advocacy, outreach, and support duties as assigned

Qualifications

Required

• Bilingual in English and Spanish is a must.
• Excellent interpersonal, communication, and relationship-building skills.
• Strong public speaking and presentation abilities.
• Passion for working with seniors and diverse communities.
• Ability to build trust and create meaningful connections with patients and families.
• Comfortable engaging with individuals one-on-one as well as in group settings.
• Valid driver's license and reliable transportation.
• Willingness to travel throughout New Jersey.
• Strong organizational and time management skills.
• Proficiency with Microsoft Office and electronic systems.

Preferred

• Previous experience in patient advocacy, patient services, healthcare, behavioral health, social services, senior services, community outreach, customer service, or a related field.
• Experience working with older adults and underserved populations.
• Experience coordinating care or supporting patients in navigating healthcare services.

Compensation

• $23.00-$26.00 per hour, based on experience, qualifications, and relevant skills.
• Part-time position, up to 30 hours per week.
• Mileage reimbursement available for eligible travel between centers on the same day.
