Position Summary 

As a Care Coordinator, you'll serve as the central connection between referral sources, patients, families, physicians, and our clinical team. You'll help coordinate the patient journey from referral through admission while ensuring timely communication, accurate documentation, insurance verification, and seamless scheduling. 

Your work helps ensure patients receive the care they need when they need it, making you an essential part of our mission to deliver exceptional home health services. 

Responsibilities 

  • Receive and process referrals from hospitals, physicians, facilities, and community partners.  

  • Verify patient demographics, insurance coverage, authorizations, physician orders, and required documentation.  

  • Coordinate Start of Care visits within required time frames.  

  • Communicate with patients and families regarding services, scheduling, and expectations.  

  • Collaborate with nurses, therapists, scheduling, billing, physicians, and referral sources to ensure seamless care coordination.  

  • Maintain accurate documentation within the electronic medical record (EMR).  

  • Monitor referral progress and proactively resolve barriers to timely admission.  

  • Assist with scheduling adjustments and coordination across clinical disciplines.  

  • Maintain compliance with HIPAA, Medicare, Medicaid, and agency policies.  

  • Provide exceptional customer service while creating a welcoming experience for every patient and referral source. 

Qualifications 

  • High school diploma or equivalent preferred.  

  • One to two years of healthcare intake, scheduling, care coordination, or administrative experience preferred.  

  • Strong organizational and multitasking skills.  

  • Excellent communication and customer service abilities.  

  • Proficiency with Microsoft Office (Outlook, Teams, Word, Excel, and PowerPoint).  

  • Comfortable learning electronic medical record (EMR) systems.  

  • Ability to prioritize competing responsibilities while maintaining attention to detail.  

  • Passion for helping others and supporting exceptional patient care.
