Position Title: PATIENT CARE COORDINATOR
DUTIES:
The Patient Care Coordinator (PCC) will be responsible for the direct coordination of all RN staff. The PCC will be responsible for problem solving and for facilitating communication between care team providers and staff. The PCC will oversee the interdisciplinary team meetings, identify various ways to improve patient care and provide leadership for employees. The PCC, when needed, will assume all or partial staff nurse responsibilities
- Coordinates and plans for departmental activities as directed by PCM and VP of Clinical Services
- Provides direction to all interdisciplinary team members to assist in meeting quality care standards, including the needs of the patient/family
- Utilizes critical thinking skills to assist staff in problem solving.
- Assists in strategic planning for department and agency
- Provides teaching to patients/families to assist in the patients comfort as needed.
- Represents and maintains the philosophy of Hospice of Huntington, Inc.
POSITION QUALIFICATIONS
- Graduate of an approved school of professional nursing with a BSN degree; advanced degree preferred.
- Licensed registered nurse in West Virginia and Ohio
- Minimum of 2 years supervisory experience.
- Minimum of 2 years health care experience; prefer home health or hospice experience.
Employment is contingent upon successful completion of a background check, drug screening, and possession of a valid driver's license.
