Overview:

Lucina is a maternal health platform that pairs AI-driven intelligence with embedded care management to deliver healthier pregnancies at lower costs. Acquired by Unified Women’s Healthcare with the backing of Ares Management and Atlas Capital Partners, Lucina is powered by an extensive women’s health dataset and a growing suite of AI-driven predictive analytics capabilities, equipping health plans with the insights and tools to identify high-risk members earlier, deliver more effective care, and demonstrate measurable clinical and financial outcomes.

Lucina is at an exciting inflection point. We are deepening our core maternity capabilities while building toward a comprehensive end-to-end women’s health platform that addresses the full lifecycle from fertility through menopause. Complemented by Unified’s affiliated provider network, virtual care capabilities, and enterprise scale, Lucina is uniquely positioned to become the only fully integrated women’s health solution for national payers and self-insured employers.

This is a rare opportunity to join a mission-driven organization at exactly the moment when strategy, resources, and market timing align.

We are seeking a compassionate, organized, and relationship-driven Patient Care Coordinator to support perinatal and postpartum patients within an OB/GYN practice model. This hybrid role works both virtually and in-person within a collaborating OB/GYN office, partnering closely with providers, clinical staff, and patients to deliver coordinated, patient-centered care across pregnancy and the postpartum period.

If you are passionate about maternal health, health equity, and supporting families during one of life’s most important transitions, this role offers the opportunity to make meaningful impact every day.

Why This Role Matters
The pregnancy and postpartum periods are critical windows for long-term maternal and infant health. As a Patient Care Coordinator, you will play a vital role in improving access, reducing barriers, and strengthening continuity of care for families during this transformative time.

Responsibilities:

  • Explain care management services and support program enrollment
  • Build trust using trauma-informed and culturally responsive communication

Care Coordination

  • Assist patients in understanding and navigating health plan benefits
  • Coordinate appointments with OB/GYNs, PCPs, pediatricians, specialists, and behavioral health providers
  • Address social determinants of health including transportation, housing instability, food insecurity, and childcare
  • Collaborate closely with OB/GYN providers and interdisciplinary teams to ensure seamless transitions of care

Patient Education & Empowerment

  • Provide education using approved materials
  • Promote self-management and patient empowerment
  • Reinforce postpartum follow-up, well-child visits, and preventive care adherence

Documentation & Technology

  • Document all encounters and care plans accurately in the EMR
  • Participate in virtual case reviews and team meetings via Microsoft Outlook and Teams
  • Maintain patient registries and support reporting initiatives
  • Complete administrative duties as assigned

Quality & Program Support

  • Contribute to quality improvement initiatives focused on maternal health outcomes
  • Support performance metrics such as postpartum visit completion and preventive care adherence
  • Adhere to organizational policies, care pathways, and safety standards
Qualifications:
  • High school diploma or equivalent required with minimum of 2 to 4 years of relevant healthcare experience in care coordination, OB/GYN practice support, or patient navigation in a hospital or medical office.
  • Associate or bachelor’s degree in a health care related field preferred. LPNs with relevant experience will be considered.
  • 2+ years of experience working with perinatal populations and/or NICU experience preferred.
  • Strong communication and interpersonal skills
  • Excellent organizational and prioritization skills
