To be considered, applicants MUST meet ALL of the following requirements:
Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), or Registered Nurse (RN) with an active certification or license (as applicable)
Minimum of 2 years of professional healthcare experience
At least 2 years of experience in one or more of the following:
Patient Care Coordination
Patient Navigation
Women's Health
OB-GYN
Care Coordination
Case Management
Medical Office
Physician Practice
Outpatient Clinic
Experience using Electronic Medical Records (EMR/EHR)
Excellent verbal and written communication skills
Strong organizational and multitasking abilities
Exceptional attention to detail and follow-through
Ability to work independently while collaborating with a multidisciplinary clinical team
Applicants who do not possess a CMA, LPN, or RN credential and at least two years of relevant healthcare experience will not be considered.
As the Patient Care Coordinator, you will play an essential role in ensuring patients receive coordinated, high-quality care throughout every stage of treatment.
Responsibilities include:
Coordinate patient care between physicians, specialists, hospitals, laboratories, imaging centers, and ancillary providers
Schedule follow-up appointments, referrals, diagnostic testing, imaging, procedures, and surgeries
Coordinate surgical scheduling, insurance authorizations, and pre-operative clearances
Monitor patient care plans from diagnosis through treatment completion
Track laboratory, pathology, imaging, and diagnostic results and communicate provider recommendations to patients
Educate patients regarding diagnoses, medications, treatment plans, surgical preparation, and post-operative recovery
Identify and resolve barriers to care, including scheduling conflicts, transportation challenges, insurance issues, and financial concerns
Conduct proactive outreach to patients who miss appointments or delay recommended treatment
Coordinate post-operative follow-up care and monitor patient recovery
Maintain accurate documentation within the Electronic Medical Record (EMR)
Escalate urgent clinical concerns to providers when appropriate
Collaborate closely with physicians, nurses, medical assistants, and administrative staff to deliver an exceptional patient experience
