Job Summary:
Tryfacta is seeking a Patient Care Coordinator for our client in Portland, OR 97216. This is a 6-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: Patient Care Coordinator
Location: Portland, OR 97216
Duration: 6 weeks (Possible extension)
Work Schedule: 07:00 AM – 07:00 PM Day Shift

Responsibilities for this position include, but are not limited to:
  • Coordinate patient care activities and serve as a liaison between patients, providers, and the healthcare team.
  • Schedule patient appointments, referrals, diagnostic testing, and follow-up visits.
  • Utilize EPIC EMR to accurately document patient information, care coordination activities, and clinical communications.
  • Assist with patient registration, admissions, transfers, and discharge coordination as needed.
  • Communicate effectively with physicians, nurses, ancillary departments, patients, and families to ensure continuity of care.
  • Verify patient demographics, insurance information, and required documentation.
  • Monitor patient flow and prioritize coordination activities to support efficient clinical operations.
  • Provide exceptional customer service while maintaining patient confidentiality and HIPAA compliance.
  • Support clinic and administrative workflows by ensuring timely completion of assigned tasks.
  • Adhere to facility policies, infection control practices, patient safety standards, and regulatory requirements.

To be considered for this position, you should have: [Skills, Education, or Experience]
  • High School Diploma or equivalent required; Associate's or Bachelor's degree in Healthcare Administration or a related field preferred.
  • Previous experience as a Patient Care Coordinator, Care Coordinator, Patient Access Representative, Unit Coordinator, Medical Office Assistant, or similar healthcare support role preferred.
  • Recent EPIC EMR experience is required (EPIC Backfill assignment).
  • Previous experience in a hospital, outpatient clinic, ambulatory care, or physician practice preferred.
  • Strong knowledge of patient scheduling, care coordination, medical terminology, and healthcare workflows.
