What You'll Do:
• Process internal and external referrals while adhering to established guidelines and policies
• Assist patients in locating healthcare and community resources
• Schedule follow-up appointments and coordinate care after hospitalizations
• Conduct 30-day referral follow-up calls and document outcomes appropriately
• Ensure referrals are properly completed and closed out
• Answer patient questions by phone and in person
• Contact patients from ER referral and discharge lists to offer follow-up appointments at CHW
• Assist with patient follow-up reporting and documentation
• Support CIHCP referral coordination and documentation processes as assigned
• Maintain accurate documentation while adhering to HIPAA and organizational policies
• Coordinate with community partners and other departments as needed
• Respond to public health emergencies and support organizational initiatives
• Carry out the mission, vision, and values of the organization
• Perform other duties as assigned
What We're Looking For:
• High school diploma or equivalent
• 1 year of experience providing comprehensive case management or social services to underserved populations
• Experience working with underserved populations in healthcare, social services, or case management settings
• Knowledge of Galveston County community resources and public assistance programs
• Familiarity with medical terminology and case management processes
• Strong customer service, communication, and computer skills (MS Office)
• Ability to work independently and manage multiple priorities
• Ability to work evenings and weekends as needed
• Valid Texas driver's license (or ability to obtain) and reliable transportation
• Must comply with GCHD immunization policy and ICS training requirements
• Must pass a criminal background check and drug/alcohol screening
Preferred Qualifications:
• Bilinugal in Spanish
• Experience coordinating referrals and follow-up care
