    Description

    CHC is a non-profit social service agency in the Akron area whose mission is to treat, inspire, support and empower individuals and families impacted by the disease of addiction. CHC has been a critical part of Ohio’s efforts to treat and prevent substance use disorders since 1974.
     
    We are looking for a Patient Care Liaison. The Patient Care Liaison is a cross-functional healthcare support role responsible for enhancing patient access, supporting clinical workflows, assisting with continuity of care activities, and maintaining accurate documentation processes within a federally aligned primary care clinic environment. This position serves as a key link between patients, providers, clinical staff, and external healthcare entities.
     
    Duties Include:
    • Front desk and patient access operations
    • Medical assistant workflows
    • Referral follow-up and tracking
    • Preventive care outreach
    • Transportation of clients, as needed
    • Continuity of care support
    • HRSA Health Center Program Requirements
    • Support accurate collection and documentation of required patient data elements
     
    Minimum requirements: Bachelor's degree in Social Work or 1+ year experience as CDCA, LCDC I or II Must have a valid driver's license and must pass a background check and drug test.
