The Patient Care Liaison is a cross-functional healthcare support professional responsible for enhancing patient access, supporting medical workflows, assisting with continuity of care activities, and maintaining accurate documentation within a federally aligned primary care clinic. This position serves as a vital connection between patients, providers, clinical staff, and external healthcare organizations to ensure a seamless, patient-centered healthcare experience. The Patient Care Liaison combines administrative, patient service, and clinical support functions while helping patients navigate healthcare services and supporting providers in delivering high-quality, coordinated care. Duties to include:
Minimum Requirements: High school diploma or equivalent required, Medical Assistant certification preferred. Minimum of 2 years’ experience in: Medical office operations Front desk registration, Clinical support or Medical Assisting. Experience in using electronic medical record systems required. Strong organizational and multitasking abilities. Exceptional attention to detail. Strong communication and customer service skills. Ability to work independently and collaboratively.
