Patient Care Manager Opportunity Description
 
Our client is a well-established Home Health organization. They have a current opening for a Patient Care Manager for their Home Health location in the the Greater Detroit Michigan area. Medicare Home Health experience as an RN or PT is required. Supervisory experience strongly preferred but may consider a strong case manager / field nurse ready to step into a management role.  Relocation candidates welcome to apply!

Patient Care Manager Job Requirements 

• Medicare Home Health experience as a Registered Nurse or Physical Therapist
• Excellent understanding of state/federal regulations, OASIS, and PDGM.
• Supervisory experience strongly preferred.

Patient Care Manager Job Responsibilities 

• Manage a team of nurses, physical therapists, and social workers.
• Ensure compliance with all state / federal regulations. 
• Train and mentor nurses.
