Patient Care Manager Opportunity Description
 
Our client is a well-established Hospice organization. They have a current opening for a Patient Care Manager for their service area in the northern Chicago, IL area. Hospice management experience is ideal but a strong hospice case manager ready to step into a supervisory role will be considered.

Patient Care Manager Job Requirements 

• Hospice experience required.
• Registered Nurse with supervisory experience.
• Excellent understanding of Medicare regulations.

Patient Care Manager Job Responsibilities 

• Manage clinical team.
• Ensure compliance with all state / federal regulations. 
• Train and mentor nurses.
• Perform field visits as needed.
