    FULL-TIME DIRECT HIRE


    Seeking an innovative and experienced leader to join our dynamic team in Philadelphia as a Patient Care Manager. In this pivotal role, you will be instrumental in enhancing the quality of patient care services delivered in a home health setting. Your expertise will guide the team in ensuring the highest standards of healthcare are met in a compassionate and patient-focused manner.

    KEY RESPONSIBILITIES:

    • Lead and manage the delivery of patient care services, ensuring compliance with all regulatory standards.
    • Provide supervision, mentorship, and support to nursing staff to promote professional development and optimal patient outcomes.
    • Collaborate with interdisciplinary teams to create and implement personalized patient care plans.
    • Ensure precise and timely documentation of patient care and progress.
    • Conduct regular audits of patient records and care plans for quality assurance.
    • Serve as a communication link between patients, families, and healthcare providers to ensure seamless care coordination.
    • Engage in continuous quality improvement initiatives and professional development activities.

    REQUIRED QUALIFICATIONS:

    • Previous experience in a home care setting with two years of management or supervisory experience, preferred.
    • Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements

    REQUIRED CERTIFICATIONS AND REQUIREMENTS:

    • Registered nurse with current licensure to practice nursing in the practicing state.
