Patient Care Manager (Home Health)
Location: St. Paul, MN
Job Type: Full-Time
Salary: $90,000–$110,000 annually (based on experience)
Schedule: Monday–Friday | 8:00 AM–5:00 PM |In Office
Are you a Registered Nurse with home health leadership experience? We are seeking an experienced Patient Care Manager to oversee clinical operations, support field staff, and help ensure exceptional patient care in a home health setting.
Responsibilities
Provide clinical oversight and supervision of patient care services
Coordinate and monitor patient care plans and service delivery
Support and mentor clinical staff to ensure quality outcomes
Ensure compliance with Medicare guidelines, agency policies, and regulatory requirements
Collaborate with patients, caregivers, referral sources, and interdisciplinary teams
Promote quality improvement initiatives and operational excellence
Foster a positive, patient-centered culture focused on compassionate care
Qualifications
Active Registered Nurse (RN) license in the state of practice
Previous home health experience required
Two or more years of management or supervisory experience preferred
Knowledge of Medicare Conditions of Participation and healthcare regulations
Strong leadership, communication, and organizational skills
