    Job Description

    Location: Perrysburg, OH

You’re passionate about guiding patients through every step of their care.

As a Patient Care Navigator at Ohioans Home Healthcare, your clinical expertise, strong assessment skills, and ability to coordinate care are key assets in supporting exceptional patient outcomes. In this role, you’ll help ensure patients receive timely, high-quality care while serving as an important connection between our patients, physicians, and clinical team.

You’ll apply your clinical judgment and nursing knowledge to support patient care, collaborate with healthcare providers, and provide guidance to our field staff when patient needs arise. We’ll rely on you to be a knowledgeable resource, respond thoughtfully to changing patient needs, and help ensure our team has the information and support needed to provide excellent care. Your ability to balance clinical priorities with our patient-first culture will play an important role in maintaining the quality experience we’re known for.

Putting the care in your career

Because we are invested in your career satisfaction, we provide:
Stability: This is a full-time, in office position.

Autonomy: Use your knowledge, skills and abilities to essentially direct the day of nursing staff and maintain the detailed daily patient care schedule.

A Culture of Respect: Employee satisfaction is important for people who provide highly personalized care. Our culture attracts the best; in fact, 90% of our staff were referred from other employees. That speaks volumes about our reputation.

Solid Support: You’ll have all the resources you need to perform at the top of your game. We promote an open-door policy with access to executive management at all times, and a team of coworkers always willing to step up and pitch in.
Excellent Training: Although autonomy is a benefit of this role, we still support your need for learning and development. That’s why we provide training and education in order for you to maintain knowledge of new staff, and to notify management of any and all staffing needs or issues.

Better Comp & Benefits: We offer competitive pay and excellent benefits that include a
401(k) plan, medical/dental/vision coverage, PTO, short-term disability, company-paid life insurance policy and much more.
