Description Nuvance Health has a network of convenient hospital and outpatient locations - Danbury Hospital, New Milford Hospital, Norwalk Hospital, and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center, and Vassar Brothers Medical Center in New York.
Purpose Performs patient care activities and non-professional services. These include addressing the personal, physical, and emotional needs of the neonate, pediatric, adolescent, adult, or geriatric patient and family. Practice must be consistent with the philosophy, goals, policies, and procedures of the Nursing Department and Medical Center administration. Population served based on scope of services in the department.
PCT II - Education and Experience Requirements
• High school diploma or GED. • AHA BLS provider certification course required within 90 days from date of hire. • Minimum of three months experience as a PCT or currently in a junior semester of an Associates or Bachelors nursing program.
Minimum Knowledge, Skills, and Abilities Requirements
• General knowledge of high school subject matter, including sciences, simple medical procedures, terminology, and office equipment.
License, Registration, or Certification Requirements
• BLS certification is required 90 days from date of hire.
Essential Responsibilities
Assists in the delivery of patient care by performing specific patient care tasks per scope of service and as directed by the Registered Nurse.
Assists with admissions, transfers, discharges, and transport of patients.
Obtains and records temperature, pulse, respiratory rate, blood pressure, oxygen saturation, weights, and blood glucose testing. Reports abnormalities and condition changes to primary nurse promptly.
Demonstrates competence in advanced skills, including but not limited to: • Oral suctioning • Enemas • No Fleets enemas • Clean dressings after a RN assessment • Tube feeding • External catheter • Condom catheter • Post-mortem care • Suction canister set-up
Communicates effectively and frequently with healthcare team members to share information and solve problems in a positive and result-oriented manner, both verbally and electronically. • Documents legibly on all hospital forms using standardized abbreviations. • Gets patient report from the RN and off-going PCT at the beginning of the shift. • Gives end-of-shift report to the primary nurse and oncoming PCT at the end of the shift. • Participates in walking rounds with the oncoming and off-going PCT at shift change.
Provides personalized care to patients and families. Identifies and documents special needs on the patient rounding sheet.
Utilizes available educational materials, programs, nursing literature, and resource personnel for career development and attends mandatory in-services to maintain unit competencies.
Utilizes material, equipment, and time in a safe therapeutic and cost-effective manner.
Assists with appropriate tasks in all code situations, i.e., CPR, obtaining equipment, etc., under the direction of the Code Blue Team.
Adheres to Organizational infection control practices and policies and to established safety and security policies, including using two identifiers whenever taking blood samples or performing any procedure.
Assists with maintaining the general cleanliness and appearance of areas on the unit, including: • Cleans treatment rooms, showers, commodes, and IV poles after use. • Cleans kitchens, refrigerators, ice machines, sinks, counters, as assigned. • Cleans counters and empties trash in nursing station as requested. • Cleans other areas, such as storage areas, linen rooms, staff lounge, as requested.
