Patient Coordinator, Initial Care Plans
Giving Home Health Care is seeking a driven, compassionate, and motivated individual to join our team as a Patient Care Onboarding Coordinator within our Case Management Department!
As a Patient Coordinator, Initial Care Plans, you will playa vital role in welcoming new clients and ensuring a smooth transition into home health care. This position involves conducting in-home visits to build trusting relationships with clients and their families, providing education about our program and their medical benefits, and initiating the startof care process.
You will also:
- Gather and organize all required compliance documents prior to the client's start of care.
- Ensure clients have the appropriate durable medical equipment (DME) and coordinate any necessary training for clients, home health aides, and nurses.
- Conduct weekly check-ins with care team members—including nurses and aides—to promote effective communication and seamless coordination of care.
- Collaborate with physicians and clients to establish and maintain personalized plans of care.
- Serve as a patient advocate, providing education, support, and consistent follow-up to ensure the highest quality experience for each client.
The ideal candidate will have excellent communication skills (both written and verbal), strong interpersonal abilities, and solid computer proficiency.

- Possession of a strong value system, showing loyalty and dedication to serving humanity
- Understand HIPAA and maintain confidentiality
- Holding a valid driver's license, car insurance, and dependable transportation for patient visits and doctor appointments
- Completion and passing of a mandatory background check by the Department of Health
Minimum Qualifications:
- Strong work ethic and relationship-building skills
- Excellent communication, interpersonal, and presentation skills
- Experience with MS Office products (Outlook, Word, Excel, etc.)
- Minimum associate level education preferred. (High school diploma or equivalent is required.)
- 1 year of medical knowledge preferred
