Patient Facilitator (81123)

$17.50-18/hour based on years of experience

Centurion is proud to be the provider of mental health services to the Pennsylvania Department of Corrections. We are currently seeking a PRN Patient Facilitator to join our Team at State Correctional Institution Cambridge Springs located in Cambridge , PA and State Correctional Institution Albion in Albion, PA

The Patient Facilitator will assist with the flow of patients in and out of the TeleHealth clinic and remain present during the clinic session. Other responsibilities will include educating the patient on the nature and process of TeleHealth, navigating and reviewing patient information using the Electronic Medical Records system, and maintaining and assisting the provider in using the TeleHealth equipment.

Shift: Day Shift 8am to 430pm (specific days would vary based on need)/ 1 day per week

• High school diploma
• Must have a current driver's license
• Must be able to pass a background investigation and obtain agency security clearance where applicable
• Ability to obtain a security clearance, to include drug screen and criminal background check.
