    Job Description

    Community Health Partnership of Illinois (CHP) is seeking a compassionate, organized, and patient-focused Patient Health Navigator to join our team in Aurora, Illinois. In this role, you will assist patients in navigating healthcare services, accessing community resources, and overcoming barriers to care while supporting CHP’s commitment to accessible, culturally responsive healthcare.

    Community Health Partnership of Illinois is a 501(c)(3) not-for-profit organization that provides primary and preventive healthcare services to uninsured and underserved communities throughout northern and central Illinois. CHP provides access to medical, dental, and behavioral health services, as well as connections to other social services.

    Responsibilities

    • Assist patients in navigating healthcare services and accessing appropriate resources.

    • Identify barriers to care and connect patients with available community and social service resources.

    • Support patients with appointment scheduling, referrals, follow-up care, and coordination of services.

    • Provide education and guidance to help patients better understand available healthcare programs and services.

    • Communicate with clinical staff, community organizations, and other service providers to support continuity of care.

    • Maintain accurate and timely documentation of patient interactions and services provided.

    • Protect patient confidentiality and comply with HIPAA and organizational policies.

    Qualifications

    • High school diploma or equivalent required; associate or bachelor’s degree in healthcare, social services, public health, or a related field preferred.

    • Previous experience in healthcare, patient navigation, community health, social services, or a related setting preferred.

    • Strong communication, interpersonal, and organizational skills.

    • Ability to work effectively with patients from diverse backgrounds.

    • Ability to maintain confidentiality and handle sensitive patient information appropriately.

    • Basic computer and documentation skills.

    • Bilingual/Spanish preferred.
