    Patient Intake Coordinator

    Location: Manitowoc, WI
    Schedule: Monday–Friday, 8:00 AM–5:00 PM
    Job Type: Full-Time

    Noble Community Clinics is looking for a Patient Intake Coordinator to join our team in Manitowoc, WI. This position plays an important role in creating a welcoming and positive patient experience while helping patients access the care and financial resources they need.

    As a Patient Intake Coordinator, you will support patients through registration, financial screening, insurance and demographic verification, and other front-office processes. You will work closely with patients, providers, and clinic staff to ensure information is accurate, patient flow is efficient, and each patient receives exceptional service.

    What You'll Do

    Patient Registration & Financial Assistance

    • Connect with patients in person and by telephone to review financial options and available resources.
    • Complete and renew Sliding Fee Scale applications.
    • Assist patients with enrollment in Medicaid and Marketplace programs.
    • Identify patients who may benefit from financial assistance and refer them to the Financial Advocate when appropriate.

    Patient Intake & Information Management

    • Check in patients for scheduled appointments.
    • Review patient paperwork for accuracy and completeness.
    • Verify and update patient demographic and insurance information.
    • Enter and maintain accurate patient information in the electronic health record (EHR).
    • Ensure required UDS data is accurately entered into the EHR.

    Front Office Operations & Patient Flow

    • Receive and accurately process patient copayments and other payments.
    • Perform front-desk responsibilities and provide excellent customer service.
    • Monitor patient flow and ensure patients and visitors are greeted and assisted efficiently.
    • Work assigned work queues and follow established workflows.

    Quality Improvement & Data Integrity

    • Support Noble's quality improvement initiatives.
    • Maintain accurate and complete patient charts.
    • Scan and upload updated patient documents into the EHR as needed.
    • Help ensure patient information is accurate, current, and complete.

    Compliance & Organizational Responsibilities

    • Maintain patient confidentiality and comply with HIPAA requirements.
    • Follow organizational policies, procedures, and Code of Conduct.
    • Maintain regular and reliable attendance.
    • Perform other duties as assigned.
    What We're Looking For

    We are looking for someone who is patient-focused, organized, detail-oriented, and comfortable working in a fast-paced healthcare environment. The ideal candidate will be able to communicate effectively with a diverse patient population and work collaboratively with providers and other members of the care team.

    Qualifications and Skills:

    • Strong analytical, problem-solving, and decision-making skills.
    • Excellent interpersonal and communication skills.
    • Ability to establish and maintain effective working relationships with patients, providers, coworkers, and the public.
    • Strong organizational and time-management skills with the ability to meet deadlines.
    • High level of attention to detail and accuracy.
    • Proficiency with Microsoft Office Suite or related software.
    Education & Experience
    • High School Diploma or GED required.
    • One year of experience working in a healthcare environment preferred.
    • Certified Application Counselor (CAC) certification required within six months of hire.
    • Basic Life Support (BLS) certification required.
