    Job Description

    Patient Placement Center Specialist supports care for Providence St Joseph Health (PSJH) patients by coordinating the patient intake process. Responsibilities include answering and triaging calls, verifying eligibility, facilitating inbound and outbound transfers, and ensuring accurate documentation. The Patient Liaison works with internal and external partners to coordinate timely patient transfers and help the team resolve complex, high-priority cases. This role supports all transfer center operations, including but not limited to the Southern, Northern, and Behavioral Health California Transfer Centers. Providence''s centralized Transfer Center coordinates patient transfers across the organization''s regional, divisional, and system healthcare network.

    Required Qualifications:

    • Associate''s Degree or equivalent education/experience.
    • 1-3 years of healthcare related experience.
    • 1 year Customer Service experience.

    Preferred Qualifications:

    • Bachelor''s Degree in Healthcare related field.
    • 1 year Call Center experience.

    Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate''s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
