Description:
Position Summary
Provide coordination and support for Mercy Patient Logistics teams to facilitate patient admissions, discharges,intra-hospital and inter-facility transfer processes. Assisting the departments to ensure appropriate placement of patients based on diagnosis and physician preference with consideration of bed availability at both acute and rural facilities. Responsible for monitoring census capacity and alerting the leadership team upon trending thresholds in various markets. Responsible for maintaining accurate bed availability information by analyzing information received on the electronic bed board and from clinical staff to coordinate patient assignments and transfers. This role is intended to help keep the flow of the Transfer Center going by having additional support for the team to obtain the information they need that at times can cause delay in the transfer process. Coordinators can help offset the high peak times by taking on the task that can ease the transfer process. This job description covers the most significant duties performed but does not exclude other occasional work assignments not mentioned.

Minimum Qualifications
Directs the daily flow of communication and collaboration between all active team members to ensure the highest level of service is being achieved.
Leads process improvement initiatives, including but not limited to, workflow development and business strategies.
Organizes key metrics into standard leader reporting.
Understands the overall strategy for Mercy One Patient Logistics (Mercy Connect Transfer Center and Patient Flow) and various service lines.
Develops strategies, in collaboration with the Mercy Transfer Team, to maintain and support team satisfaction.
Coordinates and communicates accurate, current/up-to-date census and patient bed availability information
Assists with transfer center calls and communication priorities for transfer center team.
Supports and maintains flow of incoming calls between the transfer center team.
Demonstrates knowledge and skills to log calls and communicate with customers in electronic documentation system
