Summary/Objective

The Patient Navigator - Case Management functions as a single point of contact for patients in need of resources and assistance in accessing clinical and supportive care services offered within Baptist and the community at large.

Essential Job Duties

• Assists patients in understanding their treatment options and the resources available to them within Baptist and the surrounding community as well as helping to resolve barriers to accessing primary and preventative healthcare services.
• Establishes and maintains positive working relationships within the healthcare community, as well as those business in the community at large, that can ensure patients receive the care and services needed.
• Educate patients in regards to where and when to access healthcare services including the roles played by Primary Care Physician, Urgent Care Centers and Emergency Rooms to aid them in developing strategies for improving appropriate use of these resources as well as empowering the patient and family to make autonomous health care decisions and access needed services.
• Assists in discharge planning and provides appropriate linkage with post-discharge care providers, following up with patients to ensure that they received the care and services needed as well as educating patients in the importance of adhering to post care instructions given by providers.
• Connects underfunded/uninsured patients to financial counselors or enrollers.
• Maintains appropriate documentation in regards to patient contact, referrals and services provided to patients and is familiar with the P&P governing the use of this information in the Electronic Health Record.

Required Education and Experience

Bachelor''s Degree in Social Work

1-3 years of related experience
