Patient Navigator (Clinical)

About the Role

We are seeking a compassionate, mission-driven Patient Navigator (Clinical) to join our Homeless Care Department and support individuals experiencing homelessness by connecting them to healthcare services and community resources.

The Patient Navigator (Clinical) in the Homeless Care Department plays a vital role in bridging the gap between patients experiencing homelessness and the healthcare system. This position works both in clinical settings and in the field, conducting outreach in streets, soup kitchens, shelters, and encampments to engage individuals in care, with the goal of improving health outcomes and supporting the overall well-being of individuals experiencing homelessness.

The Patient Navigator will assist clinical providers during patient visits and prepare patients for exams in accordance with established guidelines.

Central to this role is the development of professional, trusting relationships with patients. Through these relationships, the Patient Navigator meets patients where they are, both physically and emotionally, and supports them in setting and achieving individualized health goals. The Navigator serves as an advocate for patients, facilitating access to care within the health center and during external medical appointments, while also connecting them to critical community resources such as housing, food, transportation, and social services.

Key Responsibilities

• Collaborates with interdisciplinary team members (medical providers, social workers, case managers, outreach workers, and community partners) to ensure coordinated care.
• Supports medical providers as assigned.
• Operates and maintains the medical van, stocking it and ensuring clean, safe operation
• Verifies insurance coverage as indicated by CSHHC billing procedures
• Send referrals and orders (i.e. imaging test) to outside healthcare organizations as necessary, and tracks follow up.
• Follows-up with referral sources in a timely fashion as required
• Manage patients' records using electronic health records and completes appropriate patient charting.
• Accurately and legibly document vital signs, including blood pressure, pulse, respiration, temperatures, o2 saturation and finger sticks
• Performs urine dipsticks, urine pregnancy test and other CLIA waived tests when necessary.
• Maintains accurate logs of point of care testing, controls, and expirations.
• Ensures compliance with infection-control policies and procedures. Ensures all instruments and supplies are cleaned and sterilized per CSHHC protocol.
• Actively advocates on behalf of the clients with community resources.

Required Qualifications

• High School Diploma/GED required. Associate degree or bachelor's Degree preferred.
• MA or CNA certification, or EMT training required. Human Service experience is preferred. Excellent working knowledge of New Haven community social and health service agencies is preferred, Bilingual ability preferred.
• Must maintain current CPR certification at all times.
• Trauma informed Care (TIC)- Must work with patients and apply the principles of trauma informed care.
• Mental Health First Aid - Must apply the principles of mental health first aid as appropriate.
• Naloxone use Training - Must be able to identify an overdose and administer Naloxone.
• Point of Care Testing (POC) - Must provide proper point of care testing for patients. Testing includes HIV, Hepatitis C.
• Mandatory 16 Hours of Continuing education each year.
