Position Summary:
A patient navigator is a lay professional that provides resources to patients, families, and caregivers to expedite timely access to quality health care throughout all phases of Emergency Department care. The goal of the patient navigator is to work autonomously and collaboratively with the nurse manager and the multidisciplinary care team to assist in preventing fragmented or delayed care and follow up on patient concerns. The patient navigator utilizes data to collaboratively improve the Emergency Department patient experience. The patient navigator identifies, analyzes, and uses new knowledge to mitigate barriers to care.
