POSITION SUMMARY: The Patient Navigator (PN) / Licensed Practical Nurse (LPN) will assume responsibilities for the care coordination for Surgical patients. The PN works collaboratively with internal and external entities to assist in optimization of patients in preparation for surgery. The PN/LPN works with patients to address their needs by serving as the link/liaison between the patient and the rest of the care team. The PN/LPN will identify and help to close care gaps, provide support and education to the patient and family/caregivers. The PN/LPN will perform professional administrative and clinical duties including, but not limited to care, treatment, documentation, and education to maintain and support the department and to ensure efficient patient flow through the department.
