General Summary of Position
Responsible for the coordination of patient care and access to services provides materials and serves as a liaison with members of the patient's health care team as well as provides administrative support to the department physicians. Functions as a patient advocate guides patients and families throughout the process. These functions are performed in accordance with all applicable laws and regulations and Georgetown University Hospital's (GUH) philosophy policies procedures and standards.
Primary Duties and Responsibilities
Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards and safety standards. Complies with governmental and accreditation regulations.Serves as navigator for patient and family members through coordinating care with all members of the healthcare team including physicians clinic support staff and other members of the healthcare team.Coordinates patient's appointments including office specialists testing pre-operative post-operative and surgical appointments.Functions as a liaison between patients families health care team and community resources. Advocates for identifying and eliminating barriers to care.Handles referral calls to ensure streamlined access to care. Ensures patient referral process for specialty services is coordinated anticipating patient and family needs throughout the process.Provides clerical and administrative support to physicians including answering the telephones relaying messages and transmitting information to the appropriate personnel. Composes and types the correspondence of various clinical and non-clinical documents. Manage physician's calendar and coordinates meetings etc.Schedules surgical cases and admissions for the department. Communicates posting information to physician nurse and administrative staff within the department as appropriate. Communicates scheduling information including information regarding surgical schedule Georgetown University Hospital facilities policies and instructions to patients and families.Obtains and uploads into the electronic medical records all previous medical records relevant to patient care including all surgical records radiology and imaging reports and disks pathology cultures and lab work etcCommunicates referral requirements patient's financial responsibility and other pre-visit information as appropriate including necessary procedures/examination preparations e.g. NPO after midnight according to departmental policies and procedures.Appropriately and accurately scans and indexes documents using correct naming conventions into patient EHRProvides support services to patients and medical staff.Participates in meetings and on committees and represents the department and hospital in community outreach efforts.Participates in multi-disciplinary quality and service improvement teams.
