This position is in part a grant funded position and is contingent on renewed funding at the end of each award period.
JOB SUMMARY:
This role will be responsible for providing outreach and linkage-to-care services to pregnant patients and newborns throughout the first year of life. The goal of this position is to help patients access prenatal care and remain engaged in healthcare throughout their pregnancy and postpartum. Additionally, this role will assist patients with insurance enrollment, provide case management and coordination with wrap around services. This position will be housed at Chase Brexton Glen Burnie, with the expectation of travel to community events, hospitals, and other Chase Brexton locations as needed.
The expected outcome of this position is to promote appropriate use of a patient-centered medical home to promote optimal pregnancy outcomes and engage newborns in pediatric care. Other responsibilities will include: tracking patients throughout care continuum; linking new pregnant patients to Chase Brexton Health Care; and coordinating within Chase Brexton departments and externally with community partners.
MAJOR DUTIES AND RESPONSIBILITIES:
Communication
- Act as a liaison between patients, providers, hospital care managers, external programs and resources
- Communicate clearly, respectfully, and compassionately with patients and caregivers
- Accurately document medical data and patient communications
- Communicate clearly with all members of the care team within the organization and to external partners in order to convey pertinent patient issues, appointment information, provider schedules, and other information
Care Management and Coordination
- Assist patients with insurance enrollment using the Maryland Healthy Babies program
- Identify and track pregnant patients and newborns at Chase Brexton Health Care Glen Burnie and Columbia Centers and follow up on missed appointments
- Assess patients for barriers to care and coordinate linkage to internal and external resources
- Retrieve clinical documentation from multiple sources to ensure continuity of care for patients
Patient Care
- Provide education, support, and outreach to retain pregnant patients in prenatal care throughout pregnancy and postpartum; and engage newborns in pediatric care.
- Engage with pregnant patients and newborns utilizing multiple modes of outreach including attendance at community events (possibly on weekends) with the Program Manager
Analytical and Critical Thinking
- Provide perinatal care coordination and assess barriers to care and link to services to address barriers
- Link pregnant patient with pediatric services at Chase Brexton Health Care prior to delivery
Workplace Computers and Equipment
- Ensure discrete data is captured in the electronic medical record for tracking and trending
SKILLS AND ABILITIES:

EDUCATION:
- Required: Some college or equivalent work experience
- Preferred: Degree in health sciences or related field
EXPERIENCE:
- Required: One year experience working in the medical field
- Experience working with a culturally and economically diverse population
