We Are Hiring:

Location: Tulsa, OK

Facility: Ascension St. John Medical Center

Department: Total Quality Management

Schedule: Full Time, Monday-Friday, 11am-7pm

What You Will Do:
  • Serve as a critical bridge between the hospital and the community.
  • Facilitate resource navigation for patients during their emergency department visit.
  • Refer complex clinical needs to Behavioral Health or RN Care Managers.
What You Will Need:

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required
Additional Preferences:
  • Strong understanding of community and public resources available to patients who are on Medicaid or uninsured.
  • Must be able to engage patients, build trust quickly, and assess their level of motivation and need.
  • Demonstrate sensitivity to diverse populations and an understanding of unique cultural barriers to care.
  • Excellent communication skills and the ability to refer patients to appropriate Case Managers for continued care.
