Job Summary

Montefiore Einstein's New York City Region is seeking a proactive and collaborative Patient Safety Officer to join our quality leadership team and the Network Performance Group. Reporting to the Chief Quality Officer with a strong connection to the System Chief Medical Officer, this role will serve as a central figure in advancing patient safety across our organization, overseeing the work of the regulatory, quality management, peer-review, and patient safety improvement teams. The Patient Safety Officer will champion a culture of safety and continuous improvement, working collaboratively with senior leadership, the AVP of Quality clinical and operational leaders, frontline staff, and patients to ensure the highest standards of care throughout all hospital locations. The Patient Safety Officer will be responsible for the strategic direction for the patient safety program in Montefiore Einstein's NYC Region, measurably improving patient safety and reducing patient harm, and furthering a culture of safety in all areas.

Salary Range: $225,000.00-$450,000.00/year

Essential Functions

  • Lead the integration and execution of patient safety initiatives, coordinating efforts among regulatory, quality management, peer-review and patient safety improvement teams.
  • Continuously evaluate, improve, and innovate a robust patient safety program based on internal needs, external guidelines/regulations, industry best practices, and the latest evidence-based science.
  • Aggregate, analyze and interpret safety data (including serious safety events, never events, electronic trigger tools, and voluntary error reports) identifying trends, root causes, and opportunities for improvement; communicate findings and action plans to senior leadership; drive improvements to ensure best-in-class safety.
  • Ensure bidirectional communication and feedback loops with clinical departments, departmental safety leaders, front-line staff, risk management, and patients to proactively identify, assess, and mitigate safety risks, while fostering psychological safety throughout the organization.
  • Serve as a resource and advocate for patient safety best practices within the organization.
  • Drive compliance with regulatory requirements and accreditation standards related to patient safety, including National Performance Goals, the CMS Structural Measures for Safety and others, and prepare for regulatory audits and accreditation surveys.
  • Promote a culture of safety through education, training, and communication initiatives, including leading the administration of the bi-annual Culture of Patient Safety Survey and the action planning and execution of improvements following the survey and pulse surveys.
  • Develop and maintain an actionable safety dashboard and ensure critical safety interventions (e.g. daily safety huddles, safety walk rounds, Rapid Response and Cardiac Arrest improvement, and patient partners for safety) are rigorously implemented across Montefiore Einstein’s NYC Region via directing and advising the Patient Safety Improvement team and the Quality Management team.
  • Lead Montefiore Einstein’s NYC Region Patient Safety Committee, NYPORTS committee, Root Cause Analyses, Peer Review Committee, and other relevant committees.
  • Direct and advise the Quality Management team, who conducts root cause analyses, peer-reviews, voluntary error report reviews, adverse event disclosure, and Just Culture training and implementation.
  • Direct and advise the Regulatory Affairs team, who reports to the Senior Director of Quality Management.
  • Ensure best in-class standards are met for root cause analyses and other investigative processes to understand safety incidents, and implement sustainable corrective actions.
  • Provide support to associates following safety events to reduce wide-reaching impacts.
  • Provide leadership, mentorship, and development opportunities to direct reports from the regulatory, quality management, peer-review, and patient safety improvement teams.
  • Report to the Chief Quality Officer, Vice President of the Network Performance Group. Collaborate closely with Network Performance Group leaders and leaders across Montefiore Einstein’s NYC Region.
  • Connect with, update and lead with Vice-Presidents, Department Chairs, The Medical Committee of the Board of Directors, and other senior leaders across Montefiore Health System.


Qualifications

  • Doctor of Medicine, DO, PhD, DNP or other terminal degree Required
  • 10+ years of experience in healthcare settings with a minimum of five (5) years of healthcare management experience in patient safety positions Required
  • Training in safety science or evidence of extensive experience with safety and improvement efforts Required.
  • CPPS/CPHQ certifications and/or Lean Six Sigma training are a plus.
  • Ability to analyze data, develop actionable insights and drive improvement efforts.
  • Proven experience in patient safety, quality improvement, or a related field Required.
  • Strong leadership and team management skills Required.
  • Excellent communication and interpersonal abilities Required.
  • Commitment to promoting a culture of safety and continuous improvement.
  • NY State Medical license or other NY State allied health License within 180 Days Preferred
