Are you a Registered Nurse with a passion for clinical education and patient independence? Our client is seeking a Peritoneal Dialysis (PD) Home Training Nurse to join their Indianapolis team. In this specialized role, you will be the bridge between the clinic and the home, serving as a dedicated educator and case manager for patients choosing Peritoneal Dialysis.
Key Responsibilities
Specialized Training: Educate patients and their "care partners" on the mechanics of Peritoneal Dialysis, including aseptic technique, catheter care, and the use of automated cyclers.
Case Management: Manage a dedicated caseload of home patients, coordinating their monthly clinic visits, reviewing lab work, and adjusting care plans in collaboration with Nephrologists.
Environmental Assessments: Conduct initial and periodic home visits to ensure a safe, clean, and organized environment for home treatments.
Troubleshooting & Support: Act as a clinical resource for patients, providing guidance on complications (such as peritonitis prevention) and equipment issues.
Program Growth: Educate in-center patients on the benefits of home-based modalities to help them transition to a more flexible lifestyle.
Qualifications
Licensure: Active, unencumbered RN License in the state of Indiana.
Experience: Minimum of 2 years RN experience is required.
Specialized Experience: At least 12 months experience in Peritoneal Dialysis (PD) is highly preferred.
Skills: Strong pedagogical skills; you must be able to break down complex medical protocols for non-medical learners.
Travel: Ability to travel locally within the greater Indianapolis area for home assessments.
