Position Summary
The Pediatric Care Coordinator & Triage Nurse plays a critical role in supporting pediatric providers by delivering safe, timely clinical triage services and coordinating patient care across the continuum. This position serves as a key liaison between providers, patients, families, and external care partners to ensure efficient communication, accurate documentation, and a high-quality patient experience.
This role is responsible for assessing patient symptoms via phone and electronic communication, coordinating follow-up care, managing clinical documentation workflows, and supporting continuity of care initiatives. By centralizing triage and coordination activities, this position enables providers to focus on direct patient care while ensuring patients and families receive timely clinical guidance and support.
Essential Functions & Responsibilities
Clinical Triage & Patient Communication
Perform telephone triage for pediatric patients utilizing established clinical protocols, evidence-based guidelines, and nursing judgment
Assess patient symptoms and determine appropriate level of care including:
Home care guidance
Scheduling of office visits
Urgent care referral
Emergency escalation
Provide clear, compassionate, and accurate guidance to parents and caregivers
Identify and escalate high-risk or complex clinical concerns to providers in a timely manner
Document all patient interactions accurately and thoroughly within the electronic medical record (EMR)
Ensure documentation supports continuity of care and meets clinical compliance standards
Support patient education related to treatment plans, medications, and follow-up care instructions
Care Coordination
Coordinate referrals, diagnostic services, specialist appointments, and follow-up care
Track and manage care plans for patients with chronic, complex, or ongoing medical needs
Support preventative care initiatives and outreach efforts
Serve as a liaison between families, providers, specialists, and external care teams
Assist with care gap closure initiatives and population health outreach efforts
Monitor and support completion of ordered labs, imaging, and specialty consultation
Clinical Administrative Support
Manage completion and processing of school, camp, sports physical, medication authorization, and other clinical forms
Ensure accuracy, completeness, and timely turnaround of all clinical documentation requests
Coordinate provider review and signature of required documentation
Maintain organized workflows for incoming clinical requests, records, and patient documentation
Support provider message management to ensure timely response to patient communications
Operational & Workflow Support
Partner with Front Office, Medical Assistants, and Call Center teams to ensure proper routing of clinical patient concerns
Assist in maintaining efficient workflows for patient messaging, triage requests, and follow-up care coordination
Identify workflow inefficiencies and recommend process improvements to improve patient experience and provider efficiency
Support implementation of clinical initiatives and process improvements across the pediatric department
Maintain confidentiality and compliance with HIPAA and organizational policies
Qualifications
Required
Active Registered Nurse (RN) license or Licensed Practical Nurse (LPN) license in good standing will be considered
Minimum 2 years of pediatric or family medicine
Demonstrated experience performing clinical triage or patient care coordination
Strong clinical assessment and critical thinking skills
Experience working within an Electronic Medical Record (EMR) system
Strong organizational and time management skills
Excellent written and verbal communication skills
Preferred
Pediatric triage experience in outpatient or call-center environment
Experience with Epic EMR platform
Experience supporting multiple providers within a group practice
Knowledge of preventive care guidelines and care coordination best practices
Work Environment & Physical Requirements
Fast-paced clinical environment supporting multiple pediatric providers
Requires ability to prioritize effectively and respond to urgent clinical patient needs
High volume of phone and electronic communication
Requires prolonged periods of sitting, computer use, and phone communication
May require occasional standing, walking, and light lifting of materials up to 15 lbs
