Pediatric Home Health Care Manager RN

A-Line Staffing is now hiring Pediatric Home Health Care Manager RN in San Antonio, TX (Hybrid with local travel required). The Senior LTSS Service Care Manager (RN) would be working for a Major Fortune 500 Company and has career growth potential.

Pediatric Home Health Care Manager RN Highlights:

• Schedule: 8:00 AM ??? 5:00 PM, Monday???Friday (potential for overtime)
• Pay Range: $40-48 Hourly, plus mileage reimbursement

Pediatric Home Health Care Manager RN Responsibilities:

• Conduct initial assessments for pediatric members enrolled in the STAR Kids program
• Evaluate unmet needs and determine eligibility for Medicaid services based on medical conditions
• Complete approximately 6 member visits per week (typically 3 days of field visits, 2 visits per day)
• Travel from home office to members??? homes within Bexar County and surrounding areas
• Complete detailed and timely documentation with strict turnaround requirements
• Maintain compliance with training requirements and company performance metrics
• Collaborate with team members via remote tools (Teams chats, virtual support)
• Communicate effectively with families, caregivers, and interdisciplinary teams

Pediatric Home Health Care Manager RN Qualifications:

• Education: RN (ADN, BSN, or MSN required)
• Licensure: Active Registered Nurse (RN) license required
• 3+ years of nursing experience
• 2+ years of case management experience
• Home Health and pediatric experience
