JOB TITLE: Pediatric Home Health PDN (RN or LPN) - Private Duty Nurse

  • Minimum of 1-year paid experience of recent clinical pediatric care experience is required. (2+ years preferred)
    • Includes: pediatric hospital units, pediatric home health / in-home or facility care, and equivalent. 
    • Excluded: clinics/physician offices, general hospital units, behavioral health centers, and non-pediatric LTC facilities.
  • Client is dependent on care, so carefully review experience requirements below to ensure qualification before you apply.
  • Home Health Nursing experience/familiarity with an Agency is highly desired.

 

LOCATION + TRAVEL:Green Springs, OH - Seneca County

  • Willingness to travel to/from client in Green Springs, OH (Seneca County) is required.
  • Willingness to travel to clients up to at least a 30-minute radius (from your home) is required if additional hours are needed, primarily consisting of Seneca County and surrounding areas / adjacent counties.

 

EMPLOYMENT TYPE: Full-Time + Respite & Flexible Hours with desire for Long-Term Employment (2+ years with HCN)

 

SCHEDULE: IMMEDIATE availability needed; requires days + evenings (exact start + end times are customizable).

  • At minimum, this case requires 32 hrs/wk commitment with flexibility during Summer + School Year schedules. 
    • Availability for on-demand schedule flexibility and/or occasional Respite hours is highly desired.
  • Additional cases can be added for those seeking consistent Full-Time (32 hr/wk) year-round or more FT hours overall (32-40 hrs/wk) including Weekday, Weekend, and hybrid schedules.

 


Home Care Network, Inc. is seeking a highly-skilled LPN or RN with excellent in-home pediatric experience to join our home health agency as a Pediatric Private Duty Nurse (PDN) to care for a wonderful 10-year old girl in Green Springs, OH (Seneca County), with the option to add additional clients in surrounding areas if desired (not required).

 

Client Needs:

  • The pediatric patient is a sweet female child with a loving family.
    • Family is seeking a Nurse with flexibility (as able) to occasionally switch days / adjust times as needed.
  • Patient is non-mobile and requires Hoyer Lift use, G-tube care, feeding support, medication administration, pediatric respiratory management, all ADL's including showering, and assistance with communication.
  • Client is dependent on care, and staff will have to push, pull, and roll client so ability to meet all of the basic job requirements including essential physical demands is a must.
  • Effective total Care Team + Family collaboration is critical.

 

Requirements / Non-Negotiables

Job-Specific

  • Graduate of an accredited school of professional nursing.
  • Current license to practice as a Licensed Practical Nurse/Registered Nurse in the state of Ohio is required.
    • Pediatric RN preferred; LPNs with excellent experience will be considered.
  • This case requires at least 1 year of recent clinical pediatric care experience (i.e., pediatric hospital units, pediatric patient home care, etc.). This excludes pediatric experience only in clinics/physician offices, general hospital units, behavioral health centers, and non-pediatric LTC facilities.
  • Home Health Nursing experience/ familiarity highly desired.
  • Minimum of 1 year home health experience and knowledge is required in:
    • G-tube care
    • Feeding tube care
    • Medication administration
    • Hoyer lift use (present in home as patient is non-mobile)
    • Pediatric respiratory management experience
  • Ability to meet all of the basic job requirements including essential physical demands of pushing, pulling, and rolling client, and lifting/carrying up to 30–50 lbs. occasionally is a must.
  • Schedule availability + flexibility to adjust between School hours and Summer/Break hours.

General Employment

  • 18yo + active Driver's License / State ID + US Work Authorization
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • Active CPR + FA certification and documentation of both (+ proof/documentation readily available) is highly desired for speed-to-start care, but HCN will provide renewal/recertification if required.
  • Current driver's license or state ID, plus reliable transportation, are required.
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients
  • Successful completion of all HCN skills tests and competency evaluations.
  • Willingness and timely compliance to maintain all ongoing Training & Certification Requirements.
  • Good interpersonal communication skills - written and verbal.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.
  • Excellent attendance record with past employers.
  • Pro-active observation and excellent electronic documentation skills.
  • Familiarity with and discipline to ensure Rights Protection, Advocacy, and Confidentiality.

 

Compensation

LPNs: $28 - $30 per hour
RNs: $35 - $40 per hour

*Compensation is based on employee qualifications and certifications, experience, schedule and travel availability, skill specializations, positive employment history, and overall client & team fit.

Position Reports to: RN Case Manager / Program or Regional Administrator

 


Position Summary

A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.

Reports to: RN Case Manager; Clinical Administrator/Director of Nursing

 

Essential Functions & Responsibilities:

  • Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.
  • Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.
  • Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
  • Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
  • Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
  • Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
  • Recognizes and responds appropriately to potentially unsafe situations.
  • Demonstrates safe practice in the use of equipment.
  • Makes recommendations on improvement of safety.
  • Performs job in compliance with agency policies and procedures and professional and community standards.
  • Accepts responsibility for professional nursing role.
  • Follows applicable state and federal laws and agency standards.
  • Attends all meetings as required.
  • Maintains confidentiality in all aspects of the job.
  • Refrains from discussing client or staff identities, conditions, or circumstances in public or with others who do not have a legitimate interest in the person.
  • Participates in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities.
  • Performs other related duties and responsibilities as deemed necessary.

Essential Physical Demands:

  • Frequent standing, walking/bending/stooping, occasional sitting
  • Weight handled; push/pull force: 10 - 20 lbs. occasionally, 20 - 30 lbs. infrequently; carry/lift: 5 -10 lbs. occasionally, 10 - 30 lbs. infrequently.
