ESSENTIAL JOB DUTIES: • Conduct required in-home supervisory visits per NYSDOH regulations and agency policy. • Assess patient needs, environment, and Plan of Care implementation during supervisory visits. • Evaluate Home Health Aide and personal care worker performance and competency in the home. • Identify and address clinical concerns, care gaps, and safety issues; communicate findings to the care team. • Provide on-the-spot coaching and training to field staff during visits. • Document supervisory visits accurately and timely in the EMR. • Respond to clinical questions and incidents from field staff, patients, and families. • Collaborate with intake, scheduling, and operations on case starts, transitions, and care plan updates. • Participate in on-call rotation as scheduled. • Support the agency's PI/CQI program through chart audits and process improvement activities. • Other duties as assigned by leadership. AGE RELATED SKILLS & COMPETENCIES: • Oversees care delivered to geriatric, adult, and (where applicable) pediatric patient populations. • Ensures field staff competency in age-appropriate care techniques and communication. • Models culturally sensitive, age-appropriate interactions with patients, families, and staff.
