What You’ll Do
Ensure the highest quality of client care
Develop and update individualized care plans
Obtain necessary physician orders for home care cases as required by state regulations
Monitor and report changes in the client’s medical or mental condition to the attending physician and Agency care team
Supervise and support the nursing home care team
Conduct weekly nursing orientations
Conduct nursing competency assessments in the office or in the client’s home, as needed
Support the overall clinical needs of pediatric clients
Promote client safety, independence, and quality of life in the home environment
What You Bring
Current New Jersey RN license in good standing
BSN preferred
Current BLS certification
Minimum 2 years of recent pediatric clinical home care experience
Recent hands-on experience with advanced pediatric clinical skills, including:
Pediatric ventilator management
Tracheostomy care
G-tube/PEG tube care
IV infusions
Wound care
Strong clinical assessment and documentation skills
Excellent communication and time-management skills
Ability to work independently in a field-based environment
Experience supervising and supporting clinical staff
Strong attention to detail and a client-first mindset
Required Health & Immunization Documentation
Candidates must be able to provide:
Physical examination completed within the past year
PPD or QuantiFERON-TB test completed within the past year, or a chest X-ray completed within the past five years
MMR immunization within the past 10 years
