Are you an experienced Pediatric Registered Nurse looking for a flexible per diem opportunity where you can use your specialized clinical skills and make a meaningful difference in the lives of children and their families?

Affirmed Home Care is expanding its Pediatric Division and seeking an experienced Per Diem Pediatric RN to provide clinical support and assist our full-time Pediatric RN Care Manager with the oversight of pediatric home care cases throughout Fairfield County & New Haven County.

This position is ideal for a pediatric RN who enjoys hands-on clinical care, field-based nursing, independence, and providing personalized care in the home setting.

YOUR ROLE

As a Per Diem Pediatric RN, you will provide clinical support and oversight for pediatric clients by:

• Providing high-quality, individualized nursing care
• Developing and updating individualized care plans
• Obtaining required physician orders in accordance with applicable regulations
• Monitoring clients and communicating changes in medical condition to the attending physician and Agency care team
• Supervising and supporting nursing staff assigned to pediatric cases
• Conducting weekly nursing orientations
• Performing nursing competency assessments in the office or client’s home, as needed
• Providing clinical guidance and support to caregivers and nursing staff
• Supporting the ongoing clinical needs of pediatric clients
• Promoting client safety, comfort, independence, and quality of life
• Maintaining accurate and timely clinical documentation

WHAT WE'RE LOOKING FOR

LICENSE & EDUCATION

• Current Connecticut State RN license in good standing
• Current BLS certification
• BSN preferred
• Minimum 2 years of recent pediatric clinical home care experience

ADVANCED PEDIATRIC EXPERIENCE

Recent hands-on experience with:

• Pediatric ventilator management
• Tracheostomy care
• G-tube/PEG tube care
• IV infusions
• Wound care

PROFESSIONAL QUALIFICATIONS

• Strong clinical assessment and documentation skills
• Excellent communication and organizational skills
• Strong time-management skills
• Ability to work independently in a field-based environment
• Experience supervising and supporting clinical staff
• Strong attention to detail
• Compassionate, professional, and client-focused approach

REQUIRED HEALTH & IMMUNIZATION DOCUMENTATION

Candidates must be able to provide:

Physical ExaminationCompleted within the past year

TB ScreeningPPD or QuantiFERON completed within the past yearORChest X-ray completed within the past five years
