    Position Summary

    At BrightStar Care of Acton, Andover & Lowell, we believe home is the best place to heal — and compassionate, thorough assessment is the first step in delivering quality care.

    The Assessment RN conducts comprehensive in-person assessments for clients receiving homecare services. This role involves evaluating physical, emotional, and social health needs, identifying care gaps, and ensuring that individualized care plans are accurate and up to date.

    You’ll work collaboratively with our clinical team, caregivers, and families to ensure that every client receives safe, effective, and compassionate care tailored to their unique needs.

    Essential Duties & Responsibilities

    • Conduct in-person nursing assessments for new and existing clients within assigned territories.

    • Perform medication reconciliation, assess adherence, and provide education on chronic and acute conditions.

    • Take vital signs and complete relevant screenings (e.g., PHQ-9, EASI, fall risk, etc.) as appropriate.

    • Use clinical judgment to escalate urgent or significant findings to the Director of Nursing or supervising RN immediately.

    • Accurately document all findings, interventions, and care recommendations in the electronic health record.

    • Collaborate with the care team to update individualized care plans and ensure timely follow-up.
    • Experience with Wound care, Injections, Medication Administration.

    • Provide input on caregiver training needs based on client assessments.

    • Participate in case reviews and quality improvement activities.

    • Maintain compliance with all BrightStar Care policies and state/federal home health regulations.

    Qualifications

    Required:

    • Active Registered Nurse (RN) license in the Commonwealth of Massachusetts (in good standing).

    • Associate Degree in Nursing (ADN) or higher.

    • Minimum 1 year of clinical nursing experience (home health, hospital, or skilled nursing preferred).

    • Strong clinical assessment, critical thinking, and documentation skills.

    • Valid driver’s license and reliable transportation (home visits required).

    • Proficiency with electronic health record systems and Microsoft Office Suite.

    Preferred:

    • Bachelor’s Degree in Nursing (BSN).

    • Previous experience in home health or community-based nursing.
    • Bilingual in English and another language (preferred, not required).

    Working Conditions
