Per Diem CNA Home Health Aide / Certified Nursing Assistant

Overview:

The Home Health Aide (HHA) is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist. As an HHA, you will be responsible for providing various personal care services to meet the patient's needs. This includes assisting with personal hygiene, meal preparation, medication administration, and monitoring patient conditions. You will play a vital role in promoting quality, continuity, and safety in patient care.

Duties & Responsibilities:

  • Provide personal care services such as bathing, oral hygiene, and dressing
  • Assist patients with toileting activities
  • Prepare and serve nutritious meals
  • Take and record vital signs
  • Assist in ambulation and exercise
  • Administer medication under supervision
  • Maintain cleanliness and orderliness of patient's living area
  • Report patient's condition and changes to the assigned nurse
  • Adhere to documentation and care procedures
  • Attend in-service training annually
  • Participate in scheduled Case Conference meetings

Job Requirements:

  • Successfully completed a state-approved training and competency evaluation program
  • At least 18 years of age
  • Ability to read and follow written instructions
  • Self-directed with the ability to work with little supervision
  • Empathy for the needs of the ill, injured, frail, and impaired
  • Current CPR certification
  • Demonstrates tact, patience, and good personal hygiene
  • Valid driver's license and reliable transportation
