We are seeking a part-time Occupational Therapist with flexible, variable availability to support our growing home health caseload in the Twin Cities and surrounding areas. Visit volume may fluctuate based on referral flow and territory, and because we are actively building our occupational therapy program, the schedule may start out slow but will steadily grow into a consistent part-time workload. Ideally, we’re looking for 2–3 days of availability each week to meet with clients and help expand OT services across our service area.

Key Responsibilities:
Assessment, planning, implementation, and evaluation of all occupational therapy modalities to patients.
All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization.
Improves or minimizes residual physical disabilities of the patient.
Returns the individual to optimum and productive level within patient’s capabilities.
Participates with all other home care personnel in patient care planning.
Takes initial history and makes initial evaluation.
Consults with physicians regarding patient’s progress
Participates in quality assessment and performance improvement activities as assigned.
Prepares clinical and progress notes.
Helps develop the plan of care and revise, as necessary.
Consults with family and agency personnel.
Ability to do extensive bending, lifting and standing on a regular basis.
Requirements:
A graduate of a OT Program recognized by the American Occupational Therapy Association or other National accepted Council or Association
An active license to practice Occupational Therapy in MN
A minimum of four (4) years’ experience as a Occupational Therapist, home care experience a plus
Must have criminal background check
Must have current CPR certification
Must have active driver's license and clear driving record
Computer skills.
Ability to exercise initiative and independent judgment
