Pathways Healthcare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team. Position Details:
Main Office Location: Westwood, MA
Service Area: Boston, Jamaica Plain, West Roxbury, Hyde Park, Dorchester, South Boston areas.
Job Type: Per Diem
Responsibilities:
Provide skilled nursing care under the supervision of a Registered Nurse and in accordance with the plan of care.
Monitor and document patient condition, vital signs, symptoms, and responses to treatment.
Administer medications and treatments as prescribed.
Assist with implementation and ongoing evaluation of patient care plans.
Educate patients and caregivers on medications, treatments, and disease management.
Communicate changes in patient condition to the supervising RN and physician as appropriate.
Maintain accurate and timely clinical documentation.
Promote a safe and comfortable environment for patients.
Coordinate necessary supplies, equipment, and services for patient care.
Support quality improvement initiatives and compliance standards.
Accept and fulfill assigned patient visits and schedules reliably.
Qualifications:
Current and active LPN license in Massachusetts.
Graduate of an accredited practical nursing program.
Previous nursing experience preferred, especially in home health, hospice, or geriatrics preferred.
Proficient with electronic medical records (EMR).
Excellent communication and interpersonal skills.
Current BLS CPR certification.
Valid driver’s license and reliable insured vehicle required.
